Meet Absence Notification Form

Please notify the coaches at least one week prior to the swim meet to be missed.

• E-mail the details to the coaches by:
  • clicking here, which take you to the contact form. Fill in your swimmer’s name and date of absence, and will be sent to the coaches.

• Or print this form and give it to one of the coaches at the next practice.

It is extremely important that we know the attendance of each swimmer at each meet. Our coaches must send a list of swimmers to the League Administrators, who prepare the Heat rosters in advance of the meet. The coaches understand if there is an illness that is unexpected or a family emergency. If another event comes up that you are not aware of now, we must know one week ahead of the meet, if at all possible.
